that are more catastrophic in design. Plans include wellness coverage and
alternative care as well as optional prescription drug and dental coverage. |

Online Administration |

You can manage your benefits online through the
Pacific Health Trust website.
Through your own secure web page you will be able to update information for
all your benefit plans, quickly access benefit summaries, forms, provider
directories, view your premium invoices as well as enroll or terminate
employees and dependents. Be

Health Net COBRA Subsidy
Program Update (12/23/2009) COBRA Subsidy – UPDATE –
President Obama Signs Defense Bill with COBRA Subsidy Extension COBRA Subsidy Extension - The Senate passed the Department of Defense Appropriations Act of 2010 (the “Defense Act”) over the weekend and sent it to President Obama for his signature and he signed it today. So, the extension has been finalized and NLRG will provide additional details to assist with the implementation. As a reminder, some of those provisions would: · Extend subsidy eligibility to February 28, 2010; · Extend the nine month time period for the subsidy by an additional 6 months to a total of 15 months; · Permit retroactive payment of premiums for up to 60 days after enactment of the extension for those assistance eligible individuals (“AEI”) whose subsidy expired on November 30th and who did not pay their premium for December coverage. · Mandate an additional Notice to all AEIs who are on COBRA on or after November 1st or whose qualifying event is a termination of employment occurring on or after November 1st · Correct the current “glitch” that requires BOTH the termination of employment and eligibility for COBRA to be within the eligibility date. Only the date of the involuntary termination of employment will have to be on or before February 28, 2010. The COBRA period can start later. In addition, it is widely expected that the jobs bill that is being considered and is expected in January will include an additional extension IMPORTANT
NEW INFORMATION REGARDING THE COBRA SUBSIDY PROGRAM: The new law allows 60 days
from February 17th, the date it went into effect, to notify current
continuants and eligible individuals of their new subsidy privileges and
second chance election. The Pacific Health Trust is working diligently
to understand these new regulations and provide information to assist you in
understanding the requirements, meeting deadlines and answering questions as
the details of this program are determined by the key players within the
Department of Labor (DOL) and IRS. If you are a Pacific Health
Trust member who has signed up for the COBRA Administration services provided
by BenefitHelp Solutions, in the next few days you will receive specific
instructions for how they will assist in administering this program. Also,
any Pacific Health Trust member company who is COBRA eligible but not yet
